Smile Health Care is a local Home and Community Based Services (HCBS) agency, located in Minnesota. We offer support services for our older adult community, and for people with disabilities, who are eligible for waiver services. These services are meant to keep community members active and in their own homes, with their loved ones. We offer both basic, and intensive support services.
Smile Health Care serves underserved community members in the Twin Cities and surrounding areas, with quality Home and Community-Based Services. Our goal is to bridge the gap in needs by offering tailored services to enhance our community’s quality of life.

Our services are culturally competent, accessible, and designed for each person we serve. We understand that your needs don’t fit in a box, so our services don’t either. Meeting the needs of older adults and people with disabilities is our priority. We safeguard the health and well-being of our community’s most valued members.

Our Data Privacy Policy
Smile Health Care is committed to maintaining and protecting the confidentiality of the client’s Protected Health Information (PHI). Smile Health Care is required by federal and state law, including the Health Insurance Portability and Accountability Act (“HIPAA”), to protect the client’s PHI and other personal information. Smile Health Care is required to provide the client with this Notice of Privacy Practices about Smile Health Care policies, safeguards, and practices. When Smile Health Care uses or discloses a client’s PHI, Smile Health Care is bound by the terms of this Notice of Privacy Practices, or the revised notice of Privacy Practices, if applicable.
