Short-term care and intervention strategies provided to a person due to the need for caregiver relief, protection of the person or others living with the person, or the person’s need for behavioral or medical intervention. It includes the following service categories:
Assessing the person and situation to determine the factors causing the crisis
Developing a person-centered intervention plan, in coordination with the person and support team, that is based on recommendations in the assessment
Consulting with and training the caregiver to ensure successful implementation of the intervention plan
Providing ongoing technical assistance to the caregiver to implement the intervention plan
Recommending positive support strategies and revisions to the person’s support plan to prevent or minimize future crisis situations and increase the stability of the person living in the community
